#Uterin-#Fibriods
It is a benign smooth muscle growth of the myometrium. It is the most common benign uterine tumor. It can develop in a number of anatomic locations such as š
ā1- Intramural -(most common location of a leiomyoma is within the wall of the uterus).
ā2- Submucosal -(These myomas are located beneath the endometrium and can distort the uterine cavity).
ā3- Subserosal -(These are located beneath the uterine serosa).
CLINICAL FEATURESĀ ofĀ UTERINE FIBROID
1-HeavyĀ menses
2-abnormal menstrualĀ cycleĀ
3-bleedingĀ duringĀ pregnancy
4-lower back pain.
5-constipation.Ā
6-infertility.Ā Ā Ā
7-heamoroids.Ā
8-ectopic pregnancy.
9-miscarriage.Ā Ā Ā
10-postĀ partumĀ heamorrhage(PPH).
11-bleedingĀ b/w period.
DIAGNOSIS -
ā¢Pelvic examination.
⢠Sonography.
⢠Hysteroscopy.
⢠Histology.
MANAGEMENT
ā 1-Observation-Most leiomyomas can be managed conservatively and followed expec-tantly with regular pelvic examinations.
ā 2-GnRH analog therapy e.g (leuprolide,nafarelin,buserelin,goserelin)after 3-6months therapy a 60-70% reduction in size of the fibroids can be expected.
ā 3-Myomectomy- This is a surgical procedure performed if the patient desires to maintain fertility.
ā 4-Embolization.
5-Hysterectomy- (If the patient has completed her childbearing, definitive therapy is an abdominal or vaginal hysterectomy).
